Mr. Reilly is a proven operations leader and innovator with a 25-year career in health care including health plans, health systems, physician practice management, and population health. His deep and diverse operational experience coupled with extensive pharmacological knowledge brings a unique viewpoint to Chronic Care Management.

Most recently he served as the Chief Operating Officer of an innovative kidney care company, Somatus. Prior to joining Somatus, he served as a Regional President at Landmark Health. Landmark is the nation’s leading medical provider group specialized in caring for complex, chronic patients. Their doctors and providers drive to patients where they live, bringing care through house calls. This home-based medical care is designed to help patients stay well and stay home.

Mr. Reilly earned a joint pharmacy (BS) and Master’s in Business administration (MBA) from The State University of New York at Buffalo. Over his 25-year career he still makes time to practice pharmacy at least once a month in a small community drug store in the community that he grew up in caring for his parents, aunts/uncles and neighbors.
